Why 35 Ncm decides whether you get teeth the same day

Immediate loading works because the four implants are splinted together by the bridge and each one is tight enough in the bone to hold. The consensus threshold is an insertion torque of 35 Ncm or more per implant. Below it, screwing on a fixed bridge risks micro-movement and a lost fixture, so the surgeon uses a removable interim instead and the fixed teeth wait. Being told 'not today' is the plan working, not failing.

The All-on-4 trip — 4 nights, 5 days

The first trip places the implants and, where bone quality allows, fits the fixed temporary the same day. The final zirconia bridge follows on a second trip once the implants have integrated. Your surgeon sets the pace.

  1. Day 1
    Arrival, CBCT, final plan

    Transfer to Konyaaltı, CBCT, photographs and bite records. Bone quality is checked against the plan; plan and guarantee signed.

  2. Day 2
    Surgery and same-day teeth

    Extractions if needed, then four implants. If insertion torque reaches the consensus threshold of 35 Ncm or more, the in-house laboratory makes the fixed temporary and it is screwed in the same day, usually by the evening.

  3. Day 3
    Rest and bite check

    Swelling usually peaks around now. The temporary's bite is checked and adjusted; soft diet, cold compresses, prescribed medication.

    Review and hygiene coaching

If an implant or the bridge fails after you fly home

The most frequent complication in the All-on-4 consensus is the loss of a single implant, followed by peri-implant mucositis. Step one: photographs to your coordinator on WhatsApp. Step two: your treating surgeon reviews within the week and replies in writing.

Step three: a guarantee case — a fixture that has not integrated, or a bridge with a material fault — is re-treated at Tantalya at no treatment charge; travel is yours. Living with a fixed bridge — the aftercare protocol

A full-arch bridge is easy to wear and easy to neglect. The written protocol handed over at departure covers the following.

  • Soft diet while the implants integrate — typically the first weeks, your surgeon sets the timing — then a normal diet with the final bridge.
  • Cleaning under the bridge every day with a water flosser or threader floss and interdental brushes; the technique is practised in the chair on Day 4.

  • Annual digital check-up: photographs or a short video, or an intra-oral scan from a local dentist, reviewed and answered in writing by your treating dentist.
